ext/faad/gstfaad.c: Some cleanups.

Some cleanups.
Added some more debugging.
Don't ever ignore unlinked, we're not a demuxer.
* gst/qtdemux/qtdemux.c: (gst_qtdemux_add_stream):
Activate pad before adding it to the element.
